We only collect information that is reasonably necessary to provide our services and meet our professional and legal obligations.
How We Collect Information
Personal information may be collected directly from you through HotDoc bookings and intake forms, consultations, assessments, telephone calls, email, our website and other communications.
With your consent, or where otherwise permitted by law, relevant information may also be obtained from healthcare professionals or services involved in your care or your baby’s care.
Purpose of Collection
We collect and use personal information to provide postnatal, infant feeding and lactation care, assess and review your care or your baby’s care, communicate with you, arrange referrals or follow up, maintain clinical records, manage appointments and billing, and meet legal, professional and regulatory obligations.
Use and Disclosure of Information
Your personal and health information will generally only be used or disclosed for the purpose for which it was collected, for a directly related purpose you would reasonably expect, with your consent, or where required or authorised by law.
Relevant information may be shared with healthcare professionals involved in your care or your baby’s care where reasonably necessary for continuity of care, referral, clinical safety or follow up.
This may include your GP, your baby’s GP, child health nurse, maternity or hospital service, paediatrician, obstetrician, allied health practitioner, pathology or imaging provider, or another relevant healthcare professional.
Where practical, significant information sharing or referrals will be discussed with you.
We may also disclose information without further consent where required or authorised by law or where permitted in circumstances involving a serious threat to health or safety.
Evolve Postnatal Clinic does not sell, rent or trade personal information.
Information About Your Baby
Where care involves your baby, Evolve Postnatal Clinic may collect and maintain clinical information relating to your baby.
A parent or legal guardian providing information or consenting to care on behalf of a baby confirms that they have authority to do so.
Relevant information about your baby may be shared with healthcare professionals involved in their care where reasonably necessary for continuity of care, growth and developmental concerns, referral, clinical safety or follow up.
Electronic Systems and Communication
Evolve Postnatal Clinic uses electronic systems to support clinical care and practice administration.
These may include clinical and practice management software, HotDoc, Medicare related systems, electronic prescribing or referral systems, email, SMS and other secure communication services.
Information may therefore be processed or stored by third party providers engaged to support the operation of the practice.
